ACT - Team Peer Specialist
ACT is an evidence-based program that utilizes an interdisciplinary team to provide community-based psychiatric treatment, rehabilitation, and assistance to individuals battling chronic mental illness.
The ACT Peer Specialist is expected to carry out assignments to fulfill the ACT mission of providing sanctuary and support to members in the community including but not limited to:
-Providing specialty expertise to the ACT team in navigating the mental health system as a client-Regular community visits, face to face and telephone contacts in accordance with the ACT team's schedule-Housing assignments to teach independent living skills in the areas of shelter, food, clothing, medications, money, and ADLs; assisting members to find and maintain housing.
-Job placement and employment counseling-Transportation-Positive role modeling; teaching basic standards of cleanliness and safety-Training in accessing community resources-Member assessment and crisis interventions.
